SINA Welfare Trust Model Works As An Efficient Filtration System For Secondary And Tertiary Care In Government Facilities

SINA Welfare Trust aims to facilitate and provide primary healthcare services to people keeping in mind the basic financial hurdles, economic challenges, unemployment crisis and mobility issues. The services provided at the NGO include primary healthcare, authentic and quality medication, laboratory tests at par with international standards, ultrasounds, immunization, mental health counseling, hepatitis screening, and referrals to secondary health care. The NGO also provides awareness sessions at their facilities for breast cancer, HIV, Covid-19 safety protocols and more.
Understanding the urban slum populations’ struggle to make ends meet and medical care becoming a non-priority, SINA has developed an effective and efficient primary healthcare system, at a token charge of Rs. 20 to underprivileged communities and Zakat eligible patients, in order to delay the need for secondary care and restore the quality of life. SINA Welfare Trust works as an effective filtration system for overburdened public facilities by resolving a large volume of cases at primary stage. This also helps in better allocation of resources for secondary and tertiary care cases.
On its dedicated journey since 1998, the NGO today has grown with 30 operational clinics in urban slums providing services to more than 800,000 patients annually, majority of which are women and children. The financially challenged communities are scanned through a Shariah compliant Zakat evaluation system and it is deduced that 80% of the patients are Zakat eligible.
At SINA, PMDC-registered doctors undergo specialized training before being deployed and are required to adhere to a strict set of protocols for consultation and prescription. These medical practitioners are monitored and audited to ensure standard of care and consultation is maintained.
The healthcare infrastructure in the country grappling with the COVID-19 pandemic has become highly overwhelmed and saturated, and the accessibility and affordability of quality healthcare has become a matter of great concern. As of data collected in July 2020, 78% of the population continues to pay for healthcare in expensive private hospitals. Public hospitals cater to approximately 300,000 patients on a monthly basis, with long waiting hours to receive treatment and medication. To counter the volume at public hospitals and mobility issues, SINA’s clinics are situated in low-income neighbourhoods removing hurdles like travel costs and long queues. The NGO has also tapped into the doorstep healthcare services model with mobile clinics, a customized vehicle that is fully equipped to provide health services by reaching vulnerable communities.
As per the World Health Organization directive and government regulations, Pakistan’s core focus should be on primary healthcare to diagnose ailments at early stages and reduce complications of chronic diseases. For example, an early detection of diabetes that is properly managed on a daily basis reduces the risk of renal failure.
Similarly, primary healthcare plays a crucial role in reducing maternal mortality as well as child mortality. Pakistan’s Maternal Mortality Rate (MMR) stands at 186 deaths per 100,000 live births, one of the highest in Asia. SINA Welfare Trust has set a new objective of prioritizing maternal health and providing accessibility to expectant mothers with adequate care, medication and timely ultrasounds to monitor gestational health and development. The NGO also tackles the stigmatized issues of postpartum depression to provide deserving mothers with holistic care.

Japan trounces smoking epidemic with non-combustible alternatives

Lahore (Muhammad Yasir) Japan emerged as model country for tobacco harm reduction when it reduced its smoking rate by nearly a third in a span of three to four years since the introduction of non-combustible alternatives to cigarettes in 2014.

A public health expert said Japan accomplished this milestone following the introduction of heated tobacco products (HTPs).  Dr. Kumamaru Hiroya, vice director of the AOI Universal Hospital in Kawasaki, said this rate of decline in smoking incidence was not observed after years of trying nicotine replacement therapy (NTP).

He said that while NTPs such as nicotine patches proved ineffective in reducing the smoking rate in Japan, the commercial launch of HTPs like IQOS from Philip Morris International (PMI) in 2014, following a market test in 2013, led to a substantial decrease in smoking rate.

“In conclusion, after nicotine replacement therapy direction has been tried, they were not really a big success. However, ever since 2014, three heated tobacco products have been launched officially nationwide in Japan and these have been penetrating 25 percent [of total smoking population], and this product has been successful to reduce cigarette smoking in Japan so far by 30 percent in three to four years,” Dr. Hiroya, a preventive physician specializing in smoking cessation, said during a recent webinar organized by business consulting firm Frost & Sullivan.

HTPs are non-combustible alternatives to cigarettes that closely mimic the rituals of smoking minus the combustion process. These electronic devices heat, instead of burn, specially-designed tobacco units to release flavorful nicotine-containing tobacco vapor. Results of scientific studies show that tar and carcinogens found in tobacco smoke, causes the death and disease associated with smoking, and not nicotine.

In the United States, the Food and Drug Administration authorized the marketing of IQOS, the tobacco heating system of PMI, as a modified risk tobacco product (MRTP) on July 7, 2020.

The FDA authorized the marketing of IQOS in the US with the information that the IQOS system heats tobacco but does not burn it and scientific studies have shown that switching completely from conventional cigarettes to the IQOS system significantly reduces the body’s exposure to harmful or potentially harmful chemicals.

The webinar, titled “Tobacco Harm Reduction and Novel Nicotine and Tobacco Products-Evidence from the Japanese Market” focused on the experience of the Japanese market following the launch of novel nicotine and tobacco products (NNTPs), specifically HTPs in Japan since 2014.  The event was moderated by Yvonne Lucas of Frost & Sullivan.

Dr. Hiroya said HTP users account for about 30 percent among Japanese male smokers and 25 percent among female smokers.  He said dual use of cigarettes and HTPs remains low at 6.9 percent among male smokers and 4.8 percent among female smokers.

A study by Tottori University Medical School funded by Ministry of Health and Welfare also shows that the initiation to HTP use among 60,000 Japanese junior high-school students was very low at just 0.1 percent belying fears of youth uptake.

Mark Dougan, director of Transformational Health at Frost & Sullivan, said the case of Japan, being the world’s largest market of HTPs, deserves to be studied because of the significant reduction in smoking rate in the country.

“The reason we’re focusing on the Japanese market is because NNTPs have been commercially available in Japan since 2014. It’s by far the world’s largest market, and they put it to be commercially available now for seven years,” he said.

How Marium re-gained her childhood after overcoming a life-threatening disease

Lahore (Muhammad Yasir) Tuberculosis (TB) is caused by a bacterium that is known as ‘Mycobacterium Tuberculosis’. This bacteria usually just affects the lung but TB can affect any part of the body. The TB epidemic in Pakistan is extremely high and continues to rise as there is not much awareness on the disease. Pakistan ranks as the 5th most affected region by TB around the world. It is more prevalent in children due to the fact that it is not easily diagnosable since the symptoms can easily be missed or overlooked as they are very non-specific. Moreover, TB accounts for 8% to 20% of deaths of children in Pakistan every year.
Marium Mohammad Aslam is a 13 year old girl who studies in the 4th grade and resides with her family at New Karachi. She had always been a playful and bright child until 5 months ago when she started to feel sick quite often. Her symptoms included shortness of breath, sharp pain in the lungs, an extremely bad cough and a fever. After a few visits to the doctors and running some tests, Marium was diagnosed with Tuberculosis (TB) and Left-Sided Pneumothorax.
Ever since then Marium stopped taking much of an interest in sports, her grades dropped and her appetite lessened to the point where she faced unconditional weight loss. The situation got even worse when she collapsed due to an abnormally high heartbeat and facing trouble breathing. She was then rushed to a public hospital so that her treatment could be initiated but she faced a major hurdle there. Since she was diagnosed with an acute case of TB and Left-Sided Pneumothorax she had to be kept in isolation. However, Mohammad Aslam, a father of five young children, could not afford admitting Marium at a private isolation firm. Her father works as a seller at a Bachat Bazar and earns a nominal sum of PKR 20,000. He was already struggling to make ends meet at home along with the education of his children, he could simply not bear the expenses of a private health institute.
And so young Marium and her family found themselves in quite the debacle. Her parents were desperately trying to figure out alternative ways to ensure that their daughter gets the treatment that she so badly required. After facing many closed doors and failed attempts, Aslam finally learned about the work that the Patients’ Aid Foundation (PAF) has done at JPMC. With the help and support provided by the Patients’ Aid Foundation (PAF), a 13 bedded TB isolation ward for complicated TB cases and other lung related issues has been introduced at Jinnah Post-graduate medical center (JPMC).
Marium was soon shifted to one of the isolation rooms of this ward where she was accommodated and made comfortable. Her treatment was started and the doctors ruled out any other condition by running some prescribed tests such as ECHO, CT-Scan, and Ultrasound etc. She also went through a minor procedure for her Left-Sided Pneumothorax and was provided with an oxygen cylinder to assist her in breathing more comfortably. Her stay lasted for 13 days.
Mariam’s family was so grateful for this service provided by the Patients’ Aid Foundation (PAF) at JPMC. Her father was extremely thankful of how accommodating the staff was and the way they took care of his daughter and also ran various diagnostic tests which he would not have been able to afford if it weren’t for the support provided by the Patients’ Aid Foundation (PAF).
‘’I am happy to feel better, I don’t have trouble breathing and I don’t feel pain in my chest anymore. I feel like a normal and healthy kid. I am very excited to return home so I can see my mother, play with my siblings and join back school.’’ – Marium.
Marium is now almost down to the finish line on the road to recovery. She bounced back in no time and is more than ready to be back home, go back to school and go back to being her usual playful and cheery self.
